1) I have read the Parent – Athlete Handbook. As an athlete -I understand the training rules and will abide by them. As a parent- I will enforce them with my student-athlete.

2) I give permission for my son/daughter to participate in the Athletic Program at Zion-Benton High School. Check all that may apply.

3)  I understand the Alcohol/Drug Testing Policy and agree to be subject to its terms and conditions.

4)  I will participate in any required Parent/5tudent-Athlete meetings.

5)  I will pay a MANDATORY Athletic Fee of $100 prior to any tryout. This fee will be paid at the ATHLETIC OFFICE with a check (made payable to: ZBTHS) or cash. I also understand that this one-time annual athletic fee will cover the participation fee for all sports during the 2007-08 school year.

6)  Warning of Physical Risk: I acknowledge that my son/daughter may sustain injury while participating in athletics under the supervision of Zion-Benton High School and its staff, even

though proper and safe coaching techniques are implemented or utilized.

I understand the term "injury" includes a scope of mild, moderate, severe, and/or catastrophic physical ailments, harms, wounds, or consequences that may result from normal and natural participation in athletics.
